How We Perform Penile Girth Enhancement
Dr. Wierschem uses his unique GirthPro technique for penile girth enlargement. GirthPro is an innovative non-surgical procedure that utilizes a proprietary hyaluronic acid dermal filler to increase penis size. Unlike other techniques, the filler is placed while the penis is erect to ensure excellent cosmetic results in both the flaccid and erect states. Additionally, it is infused using a blunt cannula to prevent damage to blood vessels or lymphatics, which can lead to tissue breakdown and necrosis. You'll be able to see the outcome immediately in both the flaccid and erect states, ensuring your absolute satisfaction!
A GirthPro procedure involves first numbing the area with a local anesthetic, then strategically placing 2 injections around the penile shaft. We do not inject into the penile glans as it tends to prodcue unsatisfactory outcomes. After the procedure, you'll need to abstain from sexual activity for 5 to 7 days.
What is Hyaluronic Acid?
Hyaluronic acid (HA) is an FDA-approved polysaccharide naturally present in body tissues such as skin and cartilage. It has been used as a dermal filler since the early 1990s. HA has a unique ability to retain water, which swells in gel form, creating a smoothing and filling effect. HA's bioelasticity provides the penis with a natural feel in both the flaccid and erect states. Patients have experienced safe, durable results with satisfying cosmetic outcomes.
While hyaluronic acid has not been specifically FDA-approved for use in penile tissues, multiple studies have shown it to be safe. The form of hyaluronic acid used for penile girth enhancement is a heteropolysaccharide macromolecule with well-documented safety and performance characteristics. HA is the most widely used filler substance worldwide and offers many advantages over its predecessors, including safety and duration of action. Adverse events are very rare, and the majority are mild and transient, typically resolving within a few weeks without treatment.

Polymethyl-methacrylate Microspheres (PMMA)
Considered a permanent filler, this procedure involves injecting non-absorbable microspheres into the penile shaft. These microspheres are suspended in a collagen gel that eventually gets absorbed by the body, leaving the PMMA to stimulate collagen production and provide permanent volume. The procedure is performed under local anesthesia and typically takes about an hour.
While PMMA can result in significant girth improvement, over half of the patients report complications such as nodularity, ridges, irregularity, indentations or voids in tissue filling. These irregularities can affect the aesthetic outcome and tactile sensation. Infection and allergic reactions, though rare, are also potential risks.
Graft and Flap Procedures
This category includes several techniques:
- Dermal fat grafts: Fat is harvested from another part of the body and grafted onto the penile shaft to create a larger penis.
- Fasciocutaneous arterial island flaps: Tissue flaps with their blood supply are transferred from another area of the body to the penis.
- Porcine dermal acellular matrix grafts: Grafts derived from pig skin that have been processed to remove cells, leaving a scaffold that integrates with the patient's tissue.
These procedures often fail to result in a satisfactory cosmetic outcome. Common side effects include chronic pain, swelling, and loss of penile length due to fibrosis (scarring). The results are typically permanent and irreversible, meaning the patient may not be able to revert to their original state if unsatisfied with the outcome.
Subcutaneous Penile Implants
This involves the surgical insertion of a subdermal silicone implant around the penile shaft. The implant is placed beneath the skin and above the erectile tissue to increase girth. The procedure is performed under general anesthesia and takes about 1-2 hours.
Severe complications have been reported with subcutaneous penile implants, including severe edema (swelling), the formation of subcutaneous masses, penile curvature or other sexually disabling deformities, infection, non-healing wounds and significant scarring. These complications can be challenging to treat and may lead to further surgical interventions.
Corporoplastic Phalloplasty
This surgical technique involves using bilateral corporal venous grafts to expand the girth of the corpora cavernosa, the erectile tissues of the penis. The procedure is considered experimental and is performed under general anesthesia. It aims to enhance girth primarily during an erection.
Corporoplastic phalloplasty does not typically increase girth in the flaccid state and carries significant risks, including infection, fibrosis (scarring) and erectile dysfunction. As an experimental procedure, long-term outcomes and safety profiles are not well-documented.
